પાલક માતા પિતા યોજના ( Palak mata pita yojana ) અને પાલક માતા પિતા યોજના ફોર્મ ( Palak mata pita yojana form ) ગુજરાતમાં વસતા 0 થી 18 વર્ષની ઉંમરના તમામ બાળકો કે જેના માતા-પિતા હયાત નથી તેમને યોજના લાભ માળવાપાત્ર છે.
આ પાલક માતા પિતા યોજના સમાજ કલ્યાણ અને આદિજાતી વિભાગ નાઠરાવ ક્રમાંક: BCA/1078 /1755 /છતા.26/12/1978 થી રાજ્યના ચોક્કસ ક્ષેત્રોમાં અમલમાં આવતા ત્યારબાદ સામાજિક ન્યાય અને અધિકારીતા વિભાગનો ઠરાવ નંબર અનથ/102008/ન.બા/01. છતા. 29/8/2009 થી સમગ્ર ગુજરાત રાજ્યમાં અમલ માં આવેલ છે.
✥ પાત્રતાના ધોરણો
- જે બાળક કે જેના માતા અને પિતા બંને મૃત્યુ પામ્યા છે અથવા જેના પિતા મૃત્યુ પામ્યા છે અને જેના માતાએ ફરીથી લગ્ન કર્યા છે તેવા અભ્યાસ કરતાં અનાથ બાળકોની સંભાળ રાખતા નજીકના સગા,વાલી કે સંબંધીને માસિક રૂ.3000/- સહાય પેટેચુકવવામાં આવેછે. આ પાલક માતા પિતા યોજના સહાય DBT થી ચુકવવામાં આવે છે.
- પાલક માતા-પિતા યોજનામાં 0 થી 18 વર્ષની વયના બાળકોને આવરી લેવામાં આવ્યા છે જેમના માતાપિતા હયાત નથી અથવા જેમના પિતાનું અવસાન થયું છે અને જેમની માતાએ ફરીથી લગ્ન કર્યા છે.
- પાલક માતા-પિતાની વાર્ષિક આવક ગ્રામ્ય વિસ્તારમાં રૂ. 27000/- થી વધારે તથા શહેરી વિસ્તારમાં રૂ. 36000/- થી વધારે હોવાનો મામલતરદારશ્રીનો દાખલો અરજી સાથે રજુ કરવાનો રહે છે.
- પાલક માતા-પિતાએ ઉછેર માટે લીધેલ ૦૩ થી ૦૬ વર્ષની ઉંમરના બાળકોને આંગણવાડીમાં પ્રવેશ અપાવવાનો છે અને ૦૬ વર્ષથી મોટી ઉંમરના બાળકોને ફરજિયાત શાળાનું શિક્ષણ આપવાનું રહે છે.
- અરજદારના વાલીએ શાળા / સંસ્થાનું પ્રમાણપત્ર રજૂ કરવું પડશે કે દર વર્ષે અભ્યાસ ચાલુ છે.

પાલક માતા પિતા યોજનાનું અમલીકરણ જિલ્લા કક્ષાએ જિલ્લાના ચિલ્ડ્રન હોમના અધિક્ષકે કરવાનુ રહે છે.
રાજ્ય કક્ષાએથી નિયામકશ્રી, સામાજિક સુરક્ષા વિભાગ, ગુજરાત રાજ્ય, ગાંધીનગર અને જિલ્લા કક્ષાએ જિલ્લા સામાજિક સુરક્ષા અધિકારી / જિલ્લા બાળ સુરક્ષા અધિકારીની કચેરી દ્વારા. તેમજ પાલક માતા પિતા યોજના ફોર્મ અને યોજનાની સહાય મેળવવાની ઓન લાઇન અરજી ( https://esamajkalyan.gujarat.gov.in ) ના પોર્ટલ પર તેમજ તે અંગેની વધુ માહિતી માટે જિલ્લા સમાજ સુરક્ષા અધિકારીની કચેરી/ જિલ્લા બાળ સુરક્ષા અધિકારીની કચેરીનો સંપર્ક કરવાનો રહેશે.
દરેક જિલ્લાસ્તરે જિલ્લા સમાજ સુરક્ષા અધિકારીશ્રીની અધ્યક્ષ સ્થાને રચાયેલ સ્પોન્સરશીપ એન્ડ એપ્રુવલ સમિતિ (SFCAC) દ્વારા રજુ કરેલ અરજીઓની સમીક્ષા કરી સરકારશ્રીના નિયમોનુસાર પાત્રતા ધરાવતાં પાલક માતા-પિતાને સહાય ચુકવવા માટે જિલ્લા સમાજ સુરક્ષા અધિકારી દ્વારા મંજુરી આપવામાં આવે છે.

✥ Foster parents plan and foster parents plan form (Palak mata pita yojana form )
Palak mata pita yojana (Palak mata pita yojana) and Palak mata pita yojana form (Palak mata pita yojana form) are valid for all children aged 0 to 18 years living in Gujarat whose parents do not exist.
Resolution No. Anath / 102008 / N.B / 01 of the Department of Social Justice and Empowerment after the implementation of this Foster Parents Scheme Social Welfare and Tribal Department Nathrao No.: BCA / 1078/1755 / Chhata.26 / 12/1978 in certain areas of the State. Although. Implemented in the entire state of Gujarat from 29/8/2009.
✥ Eligibility standards
- A monthly allowance of Rs.3000 / - is paid to a close relative, guardian or relative caring for an orphaned child who has lost both his mother and father or whose father has died and whose mother has remarried. This assistance is paid through DBT.
- The scheme covers children between the ages of 0 and 18 whose parents do not exist or whose father has died and whose mother has remarried.
- The annual income of foster parents in rural areas is Rs. 27000 / - and in urban areas Rs. Mamlatdar's sample of more than 36000 / - has to be submitted along with the application.
- Anganwadi is to be given to the children of the age group of 04 to 06 years taken by the foster parents for upbringing and compulsory schooling is to be given to the children above the age of 08 years.
- The guardian of the applicant has to submit the certificate of the school / institution that the study is in progress every year.
✥Implementation of foster parent scheme
The implementation of the foster parent scheme is to be done at the district level by the superintendent of the district children's home.
From State level to Director, Social Security Department, Gujarat State, Gandhinagar and District level through the office of District Social Security Officer / District Child Protection Officer. Parents should also contact the District Social Security Officer's Office / District Child Protection Officer's office on the portal of the online application form (https://esamajkalyan.gujarat.gov.in) to get the scheme form and scheme assistance.
After reviewing the applications submitted by the Sponsorship and Approval Committee (SFCAC) constituted under the chairmanship of the District Social Security Officer at each district level, the District Social Security Officer approves the payment of assistance to eligible foster parents as per the rules of the Government.
