➢ મતદાર યાદીમાં નવુું નામ ઉમેરવા કરવા માટે ફોર્મ નં-૬ ભરવાનુું હોય છે. ફોર્મ ઓનલાઈન/ઓફલાઈન ભરતી વખતે નીચે મુજબ ની બાબતોનુું ધ્યાન રાખવુું.
➢ જો વિધાનસભામાં નામ દાખલ કરવુું છે ઓનલાઈન ફોર્મ ભરતી વખતે તે સાચુ પસંદ કરવુું. વિધાનસભાનુ નામ ખબર ન હોય તો કુટુંબના સભ્ય અથવા પડોશીના ચૂુંટણી કાર્ડના પાછળના ભાગમાં લખેલ વિધાનસભાનુ નામ જોઈ લેવુું. અપલોડ કરવામાં આવનાર તમામ ઈમેજ સ્પષ્ટ વાંચી શકાય તેવી હોવી જોઈએ.
➢ નામ, અટક, સંબંધિનુ નામ - ( પિતા અથવા પતિ નુ નામ ) સંબધનો પ્રકાર – (પિતા/ અથવા પતિ ) ,જન્મ તારીખ – વિગેરે.
➢ પુરાવા મુજબ બધી વિગતો સાચી લખવી.
➢ મતદાર યાદીમાં જન્મ તારીખ ,નામનો પુરાવો :- જન્મનો દાખlલો/ એલ.સી./પાનકાર્ડ/પાસ પાોર્ટ/આધાર કાર્ડ (લગ્ન પછીના જોડાણના કિસ્સામાં લગ્નનું પ્રમાણપત્ર અથવા પતિના નામ સાથેના ઉપરોક્ત પુરાવાઓમાંથી એક જોડવું જોઈએ).
➢ મતદાર યાદીમાં રહેઠાણનો પુરાવો :- હાલનુું પોતાના અથવા પોતાના ફેમીલી મેમ્બરના નામનુ લાઇટબીલ/વેરાબીલ, (ભાડાનુું મકાન હોય તો ભાડા કરાર ફરજિયાત),
➢ મતદાર યાદીમાં ફોટો :- પાસપોર્ટ સાઈઝ ફોટો જોડવો. ( સેલ્ફી ફોટો મુકવો નહી ).
➢ કુટુંબના સભ્ય અથવા પડોસીનો ચુુંટણી કાર્ડ નંબર ફેમિલી ડિટેલ્સ માં ફરજિયાત લખવો ,
➢ અગાઉના ચુુંટણી કાર્ડની ઝોરોક્ષ અથવા તેનો નંબર જો અગાઉ બનાવેલ હોય તો ડેકલેરેશન માં ફરજિયાત લખવુ. બે જગ્યાએ ચૂંટણી કાર્ડ પર નામ રાખવું ગેરકાનૂની છે. જેથી પહેલા જુની વિધાનસભા કચેરીમાંથી અથવા જુના રહેણાંક વિસ્તારના BLO (બુથ લેવલ ઓફિસર) પાસે થી નામ કમી કરાવીને જ નવી જગ્યાએ ફોર્મ ભરવુ. અથવા જુનો વોટીંગ કાર્ડ નમ્બર ફરજિયાત ઓનલાઈન ફોર્મમાં દર્શાવવુ.

Form No.6 has to be filled to enter a new name in the electoral roll (Matdar yadi) . When filling up the form online / offline keep the following in mind.
➢ If the name is to be entered in the assembly, choose it correctly while filling the online form. If you do not know the name of the legislator, look at the name of the legislator written on the back of the election card of the family member or neighbor. All images to be uploaded must be legible.
➢ Name, surname, relative's name - (father or husband's name) type of relationship - (father / or husband), date of birth - etc.

Or showing the old voting card number in the mandatory online form.
